Preventable medical errors, the 3rd leading cause of death, happen in all hospitals every day, even the highest-rated. Most are never reported. Every hospital patient is at risk.

Description: A mobile app available from the App Store or Play Store that can help patients protect themselves, and advocates help protect their patients, against preventable medical errors while in a hospital.

This app was developed in co-operation with leading medical professionals and can help hospital patients, their families, and advocates understand, avoid, and deal with preventable errors.

Communities and Businesses
This program promotes and facilitates dialogues with communities and businesses about hospital patient safety. The purpose is to reduce preventable patient harm by raising awareness and educating people about how to protect themselves from preventable harm while in a hospital.
